A specific form is not required, but it is recommended that you use Florida's standardized School Entry Exam Form. It must be completed by a healthcare provider licensed in the U.S. to perform physical exams.
Students transferring to a Florida school from another state can use a comparable form from that state as long as it has been completed within one year.
